Here's what happens if you don't help Millemog near the beginning of Biomutant's story
Biomutant has a two sided story where you can do evil things or good things. One of the first encounters of this is with Millemog, the sole survivor of a destroyed village. You can choose to not help him and something very unexpected happens.
